If you have decided to test the PCR and made a reservation by phone, please fill in the items in the red line below on the web form and send it. Please note that if you do not send it, your reservation will not be confirmed.
After you send an email from the web form, we will reply to you by email to the email address you sent. Please note that if you do not reply (attach or send by fax) the documents described in the email by the 7th day of the examination date, we may not be able to issue a medical certificate for the next medical day.
